Tooth # 18 RCT had previously been initiated by another dentist. The patient was referred to our office after the dentist encountered difficulty locating the mesial canals. The mesial canals were located without too much trouble. As I was preparing to obturate the canals, a small amount of bleeding was visible in the isthmus between the MB and ML under the microscope. After carefully inspecting the area with ultrasonics, the middle mesial canal orifice was located. While it wasn't easy to reach the apical end, with some time and effort, the treatment was successfully completed.
